1. Will my insurance cover orthotics and prosthetics? Your insurance policy is a contract between you and your insurance carrier. Our office will call your insurance company to verify orthotic/prosthetic benefits. We urge you to be fully aware of the provisions of your policy prior to your appointment. We will assist you in receiving the maximum benefits possible, and we will file a claim on your behalf. The final responsibility for payment remains with you.
2. How long will it take to get my device? Each patient receives individualized treatment. From the evaluation, we determine what your treatment plan will require and what type of prostheses/orthoses is best suited for your needs. The fabrication of this device requires impressions, drawings, measurements and models. On average, most custom orthotic devices will take one to five days to complete. Prosthetic devices may or may not take longer.
3. How do I care for my device? When you receive your device from Arise Orthotics & Prosthetics you will also receive an informational packet that explains how to utilize and care for your device. At that time, our practitioner will discuss the information with you and provide you with the opportunity to ask any questions.
4. What do I need to bring to my appointment? For your initial appointment, we ask that you arrive 10-15 minutes prior to your scheduled time in order to allow for the necessary paper work to be completed. Please bring a photo id, your insurance card and your prescription/referral. If you have Medicare or Medicaid, please be sure to have a current copy of your card with you. At the initial visit you will be asked to complete a patient information sheet which requests general information such as phone number, address, social security number, date of birth and so forth.
5. What is the warranty? All devices have a 90-day warranty. If you need an adjustment or repair in the first 90-days, please call the office for an appointment. There is no charge for adjustments or repairs during this time. After 90-days, charges may apply for adjustments and/or repairs.

7. Do you accept Medicaid? What is the Medicaid process? Yes, we are a provider for both Minnesota and Wisconsin Medicaid plans. Please let us know that you are eligible for Medicaid when you call to schedule your appointment. Please bring your current Medicaid card with you each time you visit our office. Please note, prior approval may be needed for some devices which may affect the timeframe for your care.

9. Do you accept walk-ins? Walk-ins are always welcome at Arise Orthotics & Prosthetics. We are open Monday through Friday, from 8:00 a.m. to 5:00 p.m.
